In consideration of being permitted to participate in UT7’s Youth Challenge Cup (the “Event”), scheduled to take place at Shelly Bay Field on October 14, 2026, I, as the Team/School representative completing this form, hereby acknowledge, understand, and agree to the following terms and conditions:
1. Assumption of Risk and Release of Liability
We understand and accept that participation in a youth football tournament involves inherent risks, including but not limited to sprains, fractures, concussions, heat-related illnesses, and other potential injuries.
By signing this agreement, we release, waive, and discharge UT7, its directors, officers, staff, volunteers, sponsors, and affiliated entities from any and all liability, claims, demands, or causes of action related to any injury, illness, or loss incurred by any team member during the tournament, whether occurring on or off the field.
2. Medical and Emergency Preparedness
We understand that it is the responsibility of our team to: – Obtain and maintain up-to-date emergency contact information for all participating players.
– Collect and retain any relevant medical and health information (e.g., allergies, medications, pre-existing conditions).
– Secure parental or guardian consent for each player to participate in the tournament.
– Be prepared to respond to medical emergencies until professional medical assistance is
We agree that tournament organizers are not responsible for obtaining or managing individual player health data or consent forms.
3. Insurance
We acknowledge that it is the responsibility of the team and/or individual participants to carry appropriate personal or team insurance coverage. The tournament organizers do not provide insurance for participants and are not liable for any costs related to medical treatment, transportation, or other injury-related expenses.
4. Photo and Media Release
By participating in the tournament, we grant permission to Footy Promotions and its authorized media outlets to photograph, record, or otherwise capture images and/or videos of our team members during tournament events. These materials may be used for promotional purposes, including but not limited to social media, websites, flyers, and future event marketing, without compensation or further consent.
5. Authority and Acknowledgment
By Clicking below, I confirm that I have the authority to enter into this agreement on behalf of the team, and that all players and their parents or legal guardians have been informed of, understand, and accept the terms herein.
